How Peripheral Nerve Stimulation Works
Peripheral nerve stimulation works on the gate control theory of pain — the same neurological principle underlying all neuromodulation therapies. When electrical pulses are delivered to a peripheral nerve at the appropriate frequency and amplitude, they activate large-diameter A-beta sensory fibers that "close the gate" on pain signal transmission through smaller C-fibers and A-delta fibers, preventing nociceptive signals from reaching the brain.
Unlike spinal cord stimulation — which targets the dorsal columns of the spinal cord — PNS places the stimulating electrode directly adjacent to the specific peripheral nerve responsible for the patient's pain. This targeted approach provides highly localized pain control with minimal effect on surrounding structures, making it particularly effective for discrete nerve injuries, post-surgical neuromas, and regional pain syndromes.
